Our Pledge to You

Clava Health is committed to protecting your Protected Health Information (PHI). We are required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.

How We May Use and Disclose Your PHI

For Treatment

We may use your PHI to provide medical treatment or services. We may disclose your PHI to physicians, nurses, technicians, or other personnel involved in your care.

For Payment

We may use and disclose your PHI to bill and collect payment for services we provide, or to confirm coverage with your insurance carrier.

For Healthcare Operations

We may use and disclose your PHI for activities necessary to run our practice — quality assessment, training, certification, licensing, and management activities.

To report abuse, neglect, or domestic violence
For health oversight, judicial proceedings, and law enforcement

Uses & Disclosures That Require Your Authorization

Most uses and disclosures of psychotherapy notes, marketing communications, and the sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted upon it.

Your Rights Regarding Your PHI

Right to Inspect and Copy. You have the right to inspect and obtain a copy of PHI we maintain about you, subject to certain limitations.
Right to Amend. You may request that we amend PHI we maintain about you.
Right to an Accounting of Disclosures. You have the right to request a list of disclosures we made of your PHI.
Right to Request Restrictions. You may request a restriction on how we use or disclose your PHI for treatment, payment, or operations.
Right to Request Confidential Communications. You may ask us to communicate with you by alternate means or at an alternate location.
Right to a Paper Copy of This Notice. You may request a paper copy at any time.
Right to Be Notified of a Breach. You will be notified of any breach of your unsecured PHI.

Changes to This Notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already have about you, as well as any information we receive in the future. The current notice will be available at our office and on our website.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
