Clava Health is committed to protecting your Protected Health Information (PHI). We are required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
We may use your PHI to provide medical treatment or services. We may disclose your PHI to physicians, nurses, technicians, or other personnel involved in your care.
We may use and disclose your PHI to bill and collect payment for services we provide, or to confirm coverage with your insurance carrier.
We may use and disclose your PHI for activities necessary to run our practice — quality assessment, training, certification, licensing, and management activities.
Most uses and disclosures of psychotherapy notes, marketing communications, and the sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted upon it.
We reserve the right to change this notice and to make the revised notice effective for PHI we already have about you, as well as any information we receive in the future. The current notice will be available at our office and on our website.
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
